Completes nutrition-focused physical assessments, takes measurements, reviews medical records, collects data via interviews, and evaluates anthropometric status. Evaluates assessment data to identify nutrition risks, nutrition problems, criteria consistent with malnutrition, knowledge deficits, and high-risk health behaviors. Offers nutrition interventions, orders, education, counseling, and referrals to other services. May place feeding tubes or perform indirect calorimetry studies. Provides counseling and education using evidence-based techniques that reflect appropriate health literacy and cultural sensitivity to patients with disease processes requiring nutritional management. Monitors and evaluates nutrition care plans to reach nutrition goals and promote optimal outcomes. Supervises practice experience for dietetic interns, dietetic technicians, registration eligible dietitians and new employees, participates in research, performance improvement, and initiatives. Enters complete, accurate, and timely electronic health record documentation. Enters nutrition orders when authorized by Medical Staff policy and procedure to reflect patient care. Participates in patient care rounds, collaborates with physicians, nurses, and other allied health professionals to create nutrition plans of care. Promotes evidence-based nutrition care and makes expertise available to foodservice areas. Provides timely responses to internal electronic communication. • Avails self to staff on units as the nutrition expert [Required]
• Performs Meal Test Trays 3 times per month (breakfast, lunch, dinner) [Required]
• Attends interdisciplinary meetings and rounds on assigned units as scheduled unless valid excuse demonstrated [Required]
• Assists in regulatory survey preparation when applicable [Required]
• Reviews and/or updates department resource/reference materials when applicable or required [Required]
• When confronted with barriers to meeting patient care standards, improves process by planning and implementing action steps [Required]
• Identifies specific patient or staff needs, initiates and develops educational or clinical materials then evaluates outcomes [Required]
• Assures that care delivered to each patient is in accordance with established standards [Required]
• Skill and knowledge in working with an electronic health record system [Required]
• Analyzes and interprets reliable assessment data to identify an actual occurrence, risk of, or potential for developing a nutrition problem [Required]
• Nutrition Diagnoses/Problems are included on the patient care plan and updated as more data becomes available, or signed off as applicable [Required]

• Complies with National Patient Safety Goals regarding abbreviation usage, patient identification [Required]
