Now Hiring: Social Services Assistant/Receptionist - Fulltime

Schedule: Tuesday - Saturday 8:30am - 5:00pm

About the Role:

The Social Services Assistant / Receptionist is a dual-role position, Monday through Friday. During morning hours, this position supports the Social Services Director with resident psychosocial care, admissions, care planning, discharge planning, and documentation, including participation in the daily morning meeting. During afternoon hours through the close of business, this position is directly responsible for greeting visitors, gathering personal information from inquiries, answering telephones, taking/delivering messages and responding to community supervisor directions.

Key Responsibilities:

Social Services Support (Primarily Mornings)

  • Attends and participates in the daily morning meeting.
  • Assists the Social Services Director with new admissions, including welcoming residents and families, completing admission paperwork, and gathering information for psychosocial histories and assessments.
  • Schedules and helps coordinate care plan conferences; notifies residents, families, and interdisciplinary team members and assists with related documentation.
  • Assists with discharge planning activities, including coordinating follow-up appointments, home health services, equipment, and transportation, as directed by the Social Services Director.
  • Documents in the resident medical record in an accurate and timely manner, in accordance with facility policy and state and federal regulations.

Reception (Primarily Afternoons, Until Close of Business)

  • Greets all visitors, gives tours and assists as needed.
  • Maintains records of resident sign in and sign out. Conducts and records visitor health screenings asrequired by Community policy.
  • Answers the telephone in a polite and courteous manner. Forwards calls, takes messages and delivers those messages to appropriate personnel.
  • Completes direct inquiry sheets on prospects.
  • Works with other staff to maintain continuous coverage of front desk, telephone answering, taking and delivering messages.

Requirements

  • High School Diploma or equivalent, required.
  • Prior experience in social services or case management within a healthcare setting, preferred.
  • Minimum of 2 years related experience, preferred.
  • Clerical and telephone answering skills, required.
  • Experience in Skilled Nursing, Assisted Living and/or Multi level care facilities, preferred.
  • Basic knowledge on Microsoft Office programs (Outlook, Word, Excel), required.
  • Attends and participates in continuing educational programs.
