What You’ll Do 

This role will be critical in designing, operationalizing, and scaling clinical programs that address core clinical needs in our patient population. This individual will serve as the program management lead, reporting to the Director of Total Cost of Care and working in close partnership with a Medical Director who will serve as the clinical leader for this work. This role requires a strong background in healthcare program management, with the ability to bridge clinical strategy and operational execution. This Enterprise, Total Cost of Care Lead will design clinical workflows, build and track key performance indicators, and ensure successful implementation across market teams. 

The Day to Day 

  • Identify top Total Cost of Care (TCOC) opportunities and evolve Strive’s clinical programs using claims, utilization, and care management data. 
  • Collaborate with the Director of Total Cost of Care and dedicated Medical Director to design and launch clinical programs aligned with organizational goals. 
  • Translate clinical objectives into operational workflows that can be scaled across markets. Create program playbooks and toolkits to support scaling and replication. 
  • Manage timelines, deliverables, and cross-functional stakeholders for clinical initiatives. 
  • Partner with training and market leadership to ensure consistent adoption and implementation into market workflows. 
  • Partner with analytics teams to define, track, and report on key leading and lagging indicators of program success (e.g., CHF admissions, medication adherence, patient outcomes). 
  • Use data to drive continuous improvement, adjust workflows, and report results to leadership. 

Qualifications 

Minimum  

  • Bachelor’s Degree in a relevant field. 
  • 4+ years in clinical strategy and clinical workflow development in value-based care or population health environments leading Total Cost of Care, care transformation, or medical expense reduction programs. 
  • Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms. 
  • Ability to travel and be onsite to meet business needs. 

 

Preferred  

  • Experience in clinical program design or clinical background strongly preferred. 
  • Demonstrated experience leading cross-functional teams and initiatives. 
  • Strong analytical acumen—ability to interpret claims/utilization data and guide decision-making. 
  • Fluency in project management, initiative planning, and performance tracking with operational follow-through. 
  • Financial literacy—understanding of medical cost drivers and VBC incentive structures.

  • Strong problem-solving skills with the ability to thrive in a fast-paced, dynamic environment.  
  • Passion for advancing value-based care and improving outcomes for patients with chronic kidney disease. 
  • Proven ability to foster collaboration across multiple teams and departments to drive results. 

 

 Annual Base Salary Range: $86,000 - $104,000
