JOB SUMMARY 

The Medication Therapy Management (MTM) Clinical Program Pharmacist will provide patient-centered MTM services for the purpose of optimizing medication therapy to improve clinical, economic, and humanistic outcomes. The MTM Program Pharmacist will participate in a broad spectrum of MTM to help achieve these outcomes.  MTM services may be provided to patients telephonically or by remote video link (telehealth).

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Deliver MTM services, including but not limited to, Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions in Care.
  • Conduct proactive patient outreach to introduce and explain the benefits of the MTM program, while overcoming objections and using persuasive communication techniques to engage patients who may be hesitant or initially unreceptive.
  • Receive and respond to incoming patient calls; take appropriate action based off protocols.
  • Accurately document all discussions with patients.
  • Communicate and document clinical recommendations to prescribers and interdisciplinary care teams.
Qualifications

KNOWLEDGE & REQUIREMENTS

Clinical Knowledge & Decision Making
•    Skilled in assessing medication therapy for appropriateness, effectiveness, safety, dosing accuracy, unnecessary or missing therapies, adverse drug reactions, and adherence concerns.
•    Proficient in developing evidence based interventions to resolve medication therapy problems, achieve therapeutic goals, and prevent future issues.
•    Knowledgeable in cost effective medication strategies that support responsible medication utilization.
•    Experienced in identifying adherence barriers and creating practical, patient specific solutions.
•    Strong clinical therapeutic expertise, including appropriate medication selection and dosing for adult and geriatric populations.
•    Adheres to state pharmacy laws, CMS program requirements, HIPAA standards, and internal privacy and compliance guidelines.
•    Maintains active pharmacist licensure in required state(s) based on business needs.
Communication & Interpersonal Skills
•    Speaks clearly and effectively with senior citizens, caregivers, and healthcare professionals.
•    Demonstrates warmth, empathy, and patience through active listening and supportive communication.
•    Communicates complex information clearly and professionally, both verbally and in writing, to diverse audiences.
•    Maintains a consistent professional demeanor in all interactions.
•    Uses effective interviewing techniques to gather accurate patient information, concerns, and needs.
•    Applies strong conflict resolution skills to deescalate situations, manage objections, and maintain rapport.
Collaboration & Patient Engagement
•    Works effectively with prescribers, interdisciplinary teams, patients, and caregivers to establish individualized therapy and disease state goals.
•    Provides patient centered education on chronic disease states and on preventive and therapeutic medication regimens.
Technology & Documentation
•    Troubleshoots basic technical issues involving internet connectivity, software navigation, and peripheral equipment.
•    Efficiently navigates MTM platforms and multiple clinical databases or applications.
•    Proficient with Microsoft Office applications, including Excel, Word, and Outlook.
•    Produces accurate, concise, and timely clinical documentation of assessments, prior reactions, recommendations, and care interactions.

•    Demonstrates strong organization, self motivation, attention to detail, and the ability to work independently in a collaborative environment.
•    Follows company and departmental procedures consistently and reliably.
•    Excels in multitasking within dynamic workflows and managing responsibilities autonomously.

EDUCATION & EXPERIENCE REQUIREMENTS

Required:

•    Minimum years of work experience:  1 year
•    Minimum level of education or education/experience:   Bachelors Degree in Pharmacy or PharmD from a college accredited by ACPE or graduate of a foreign college of pharmacy
•    Current license to practice Pharmacy in the state(s) you are practicing in 
•    Ability to acquire pharmacist licensure in multiple states, if requested
•    Completion of an MTM certificate course 

Preferred:
•    Board Certification in Ambulatory Care, Geriatrics, Pharmacotherapy, Medication Therapy Management, or other Board Certification provided through American College of Clinical Pharmacy 
•    Completion of an ASHP accredited pharmacy residency and/or fellowship
•    Pharmacist licensure in multiple states
•    Bilingual and Fluent in English & Spanish/Mandarin/other language, highly preferred.
•    Motivational Interviewing Skills

The hiring rate for this role is $54.00 per hour. There may be a bonus or commission, and/or a long term incentive that will be provided as part of the compensation package, along with medical, financial and/or other benefits, depending on the level and position offered.
