The Clinical Medication Therapy Management (MTM) Pharmacist will provide patient-centered MTM services for the purpose of optimizing medication therapy to improve clinical, economic and humanistic outcomes. The Clinical MTM Pharmacist will participate in a broad spectrum of MTM services including, but not limited to, Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions in Care to help achieve these outcomes. MTM services may be provided to patients telephonically or by remote video link (telehealth)

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Deliver MTM services, including but not limited to, Comprehensive Medication Reviews, Disease Specific Medication Reviews, and Targeted Interventions to assess appropriateness of all medications, to optimize medication therapy and to improve clinical outcomes
  • Communicate and document clinical recommendations to prescribers and interdisciplinary care teams
Qualifications

KNOWLEDGE & REQUIREMENTS

  • Knowledge and understanding of the 5 Core Elements of MTM services – 1) Medication Therapy Review – Comprehensive and Targeted, 2) Personal Medication List (PML), 3) Medication-Related Action Plan (MAP), 4) Interventions and Referrals, and 5) Documentation of Services and Follow-up
  • Ability to efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns
  • Ability to efficiently propose appropriate interventions to resolve medication therapy problems, achieve goals of therapy and prevent new medication therapy problems
  • Ability to assist with managing medication costs through cost-effective interventions 
  • Ability to help patients identify medication adherence barriers and develop solutions to overcome those barriers
  • Ability to efficiently evaluate and accurately document patient care notes, prior adverse reactions, patient recommendations, and prescriber recommendations in a clear, concise, professional and timely manner
  • Ability to collaborate with prescribers, interdisciplinary care teams, patients and/or caregivers to identify and develop patient-specific disease state therapy goals
  • Ability to provide patient-centered education on chronic disease states, including medications used for prevention and treatment
  • Strong Telephonic Skills – Ability to speak clearly and communicate fluidly with senior citizens and healthcare professionals alike
  • Excellent Listening and Interpersonal Skills – Ability to convey warmth and empathy to others while patiently employing active listening technique
  • Clear and Concise Communication Skills – Ability to articulate complex information, both verbally and in writing, in a clear and concise professional manner to a variety of audiences

  • Exceptional Interviewing Skills – Ability to probe and question patients to extract information to understand concerns and needs
  • Conflict Resolution Skills – Ability to defuse potential customer conflict and respond supportively to patients through use of appropriate tone, questioning, and/or counsel

  • Self-management Skills – Self-motivation, organization, and attention to detail
  • Knowledge of the Law – Abide by state pharmacy law and understanding of basic compliance measures for the CMS programs, and ensure privacy and confidentiality as required by HIPAA, company and  department guidelines
  • Maintain Licensure – Maintain active pharmacist licensure in state(s) required for business needs
  • Computer/Technology Competency:
    • Ability to efficiently navigate approved MTM platforms
    • Proficient in use of MS Office applications, including Excel, Word, and Outlook
    • Ability to multi-task using multiple databases/platforms/applications
  • Strong clinical therapeutic skills and demonstration of knowledge of appropriate medication dosing in geriatric and adult populations
  • Proven ability to work independently in a collaborative environment, be self-directed 
    and possess excellent organizational skills

EDUCATION & EXPERIENCE REQUIREMENTS

  • Minimum years of work experience: 1 year
  • Minimum level of education or education/experience: Bachelors Degree in Pharmacy or PharmD from a college accredited by ACPE or graduate of a foreign college of pharmacy
  • Current license to practice Pharmacy in the state(s) you are practicing in 
  • Ability to acquire pharmacist licensure in multiple states, if requested
  • Completion of a MTM certificate course

Preferred: 

  • Board Certification in Ambulatory Care, Geriatrics, Pharmacotherapy, Medication Therapy Management, or other Board Certification provided through American College of Clinical Pharmacy 
  • Completion of an ASHP-accredited pharmacy residency and/or fellowship
  • Pharmacist licensure in multiple states
  • Motivational Interviewing Skills

The hiring range for this position is $XX to $XX per hour. The actual base pay offered will consider candidate’s geographic location, job related knowledge, experience, and skill amongst other potential factors. There may be a bonus or commission, and/or a long-term incentive that will be provided as part of the compensation package, along with medical, financial and/or other benefits, depending on the level and position offered.
