Position Summary
The Senior Manager, Community Engagement and Strategic Partnerships, will report to the Lead Director, Program Evaluation and Quality Improvement, Health Engagement and Access Team (HEAT) office. This detail-oriented leader is responsible to enable equitable, coordinated care by strengthening community and provider ecosystems, improving care navigation and equipping the workforce to deliver integrated, culturally responsive care that improves outcomes and social impact.
The incumbent for this role needs to build and steward trusted relationships with communities, providers, and partners. We're looking for a leader with excellent communication skills and be able to drive and influence strategic partnerships across the enterprise. This leader should have proven expertise in increasing member and provider engagement, including primary care adherence and digital engagement.
1. Identify opportunities, ideate & design interventions for equitable coordinated care within community & provider ecosystems including closed loop referral opportunities.
2. Create models for and execute on opportunities to enable community workforce to deliver integrated, culturally responsive care for better health outcomes
3. Build and steward trusted relationships with communities, providers and partners.
4. Build and maintain relationships with community-based organizations, provider groups, and social services to co-design interventions and address upstream barriers.
5. Identify and build opportunities to increase member and provider engagement, including driving primary care adherence and digital engagement.
6. Ensures enterprise colleagues and community workforce have the necessary skills, tools, and confidence to support equitable care navigation (training, education, etc.)
Required Qualifications
8-10 years of prior relevant work experience.
Population health program implementations.
Demonstrated experience in building interventions to address upstream barriers for better health outcomes.
Prominent level of interpersonal skills and ability to manage multiple business areas, community and provider partners.
Coordinates and prepare for external community partners and provider meetings
Ensures that the most impactful internal subject matter experts (clinical, regulatory, financial, analytical, etc.) are utilized to optimize performance.
Experience developing analyses and presentations to explain business issues, quantify financial impact, and provide recommendations
Exceptional oral, written, and presentation communication skills.
Attention to detail and proactive problem‑solving.
Proven ability to interact with, influence and collaborate with internal and external stakeholders at all levels,
Experience managing matrixed environment with ability to leverage internal business partners to complete intervention design
Preferred Qualifications
- ACO/Managed Care / Value based care models experience
- Sound risk‑mitigation judgment
- Communicate effectively and succinctly with a diverse range of stakeholders, both verbally and on paper, and within all levels of the organization.
- Effective decision making.

Bachelor's degree required
Masters preferred
Pay Range
The typical pay range for this role is:
$75,400.00 - $182,549.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company’s equity award program.
