Since 2005, MDCalc has been an essential part of clinicians’ practice. Often introduced during medical school or residency, MDCalc has become one of the most trusted and used clinical decision support platforms; used by more than 65% of physicians worldwide. Trust, authority, and accuracy fuel a portfolio of over 800 tools, scores, calculators, and assessments that clinicians of over 50 specialties rely on to help them achieve better outcomes for over 200 patient conditions. This wide use and integration within the clinical workflow has established MDCalc as a preeminent HCP platform and a fixed resource at the point of clinical decision-making.
To continue to further accelerate and steward this growth, we are seeking a Senior Full Stack Engineer on our expanding tech team. This role will be critical to MDCalc’s expanded success in continuing to support our millions of clinical users worldwide in taking care of hundreds of millions of patients.
The RoleAs a Senior Full Stack Engineer on MDCalc's commercial team, you'll be the technical backbone of the systems that power our revenue and business operations, the infrastructure that makes MDCalc's commercial partnerships work at scale. You'll build and own internal tooling, data pipelines, and backend services in close collaboration with our data, product, and commercial teams.
Design, build, and own backend systems and internal tools that support advertising delivery, campaign management, audience segmentation, and commercial reporting
Develop and maintain robust data pipelines and analytics infrastructure to power commercial decision-making
Partner with business stakeholders to translate commercial requirements into scalable technical solutions, writing clear specs, querying data, and iterating on internal dashboards and tools
Contribute to architecture decisions around data modeling, API design, and system performance for commercial-facing services
Ensure data accuracy, integrity, and observability across commercial systems through thoughtful instrumentation, testing, and documentation
Collaborate across engineering, product, and revenue teams to ship reliable, well-documented software with real business impact
Bachelor's or Master's degree in Computer Science, Engineering, or a related field (or equivalent experience)
Strong full stack proficiency with a clear backend lean. You're fluent with Node & React/Next.js but your instincts and excitement live in server-side systems, APIs, and data
Strong experience with relational databases (Postgres) and data modeling; you write complex queries confidently and think carefully about schema design
Experience building internal tooling, data pipelines, or analytics infrastructure, ideally in a commercial, ad-tech, or revenue-adjacent context
Comfort moving between engineering and data roles: as confident writing a migration script as pulling insights from a dataset
Experience with Amazon AWS and modern infrastructure tooling (Docker, Terraform) is a plus
Strong communication skills and the ability to translate technical tradeoffs for non-technical stakeholders
A background in ad tech, commercial platforms, or data-heavy engineering is a meaningful plus
Ability to make a true difference in medicine: MDCalc is the most broadly used medical reference by physicians, used by over 65% of US attending doctors weekly
Medical, Dental, & Vision Coverage, with option to extend to your dependents
Company-sponsored short-term insurance
Fully-paid 8 week parental leave, after 6 months of employment
Company-sponsored 401k, after 3 months of employment
Unlimited vacation for salaried roles - we trust you to take the time you need
Tri-annual company offsites to connect, reflect, and plan together (most recently in Asheville and Mexico City)

Hybrid work environment with a great team office in Greenwich Village, NYC
A culture of fun and motivated team members who believe in a greater mission here at MDCalc
