At DaVita, we are transforming kidney care. As the Sr. Manager, Patient Activation, you will play a critical role in shaping how patients engage in their care journey -helping them adopt evidence-based care plans, access preventive and disease management programs, and ultimately improve health outcomes while reducing the total cost of care.

This is a unique opportunity to lead a new strategic lane within our Integrated Kidney Care (IKC) business, supporting patient populations across DaVita. Success in this role requires not just strategic design, but also the ability to navigate a complex, multi-stakeholder environment -bringing together clinical, operational, product, and field teams to co-create scalable solutions. What You’ll Do

  • Lead Patient Activation Strategy – Serve as the program lead for ESKD patient activation, ensuring initiatives align with enterprise goals and quality outcomes.

  • Design & Deliver Programs – Develop, pilot, and scale activation approaches that improve key outcomes (e.g., transition-of-care adherence, lifestyle adoption, treatment consistency).

  • Create Meaningful Content – Partner with marketing and clinical teams to design patient-facing education, outreach scripts, and tools.

  • Drive Cross-Functional Alignment – Act as the connective tissue across Operations, Clinical Transformation, Product, and Field teams to ensure strategies are feasible, scalable, and supported by all stakeholders.

  • Influence Through Collaboration – Build strong relationships and influence decision-making at multiple levels of the organization, ensuring buy-in and shared ownership of activation goals.

  • Track & Measure Performance – Define key metrics (e.g., activation rates, self-management adoption) and report outcomes across markets and contracts.

  • Manage Projects & Partnerships – Oversee pilots, enterprise rollouts, and vendor/community collaborations that enhance patient activation.

  • Innovate & Improve – Apply behavioral science, digital engagement tools, and best practices to continually elevate the patient experience.

  • Champion Change – Advocate for activation strategies across the Village, ensuring field teammates understand and support patient activation efforts.

What We’re Looking For

  • Education: Bachelor’s degree required; Master’s in Public Health, Health Administration, or related field preferred.

  • Experience: 7+ years in patient engagement, population health, behavioral science or healthcare program management.

  • Expertise: Proven ability to deliver results in highly cross-functional, multi-stakeholder settings; experience in value-based care or chronic disease management preferred.

  • Skills: Exceptional communication, facilitation, and storytelling skills; data-driven mindset with a track record of defining and achieving measurable outcomes.

  • Mindset: Strategic thinker and natural collaborator who thrives in ambiguity, builds trust across teams, and brings people together to achieve bold goals.

This position will be open for a minimum of three days.

The Salary Range for the role is $82,800.00 - $121,600.00 per year.

If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.

New York Exempt: New York City and Long Island: $64,350.00/year, Nassau, Suffolk, and Westchester counties: $64,350.00/year, Remainder of New York state: $60,405.80/year New York Non-exempt: New York City and Long Island: $16.50/hour, Nassau, Suffolk, and Westchester counties: $16.50/hour, Remainder of New York state: $15.50/hour

Washington Exempt: $77,968.80/year Washington Non-exempt: Bellingham: $17.66/hour, Burien: $21.16/hour, Unincorporated King County: $20.29/hour, Renton: $20.90/hour, Seattle: $20.76/hour, Tukwila: $21.10/hour, Remainder of Washington state: $16.66/hour

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
