Mountain Vista Senior Living Community is seeking a Director of Social Services to lead the Social Services department within our well-established senior living community. This leadership role is responsible for ensuring residents receive comprehensive psychosocial support while maintaining compliance with state and federal regulations.

What You’ll Do
- Lead the Social Services department and oversee psychosocial care for residents across the community
- Develop and implement policies, procedures, and standards of practice for social services
- Conduct psychosocial assessments and provide counseling and supportive services to residents
- Advocate for residents’ rights, well-being, and quality of life
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Participate in discharge planning and coordinate transitions of care
- Serve as a liaison between residents, families, community agencies, and healthcare providers
- Support families through adjustment, transitions, and care planning decisions
- Provide crisis intervention when needed
- Maintain accurate documentation of social work assessments and interventions
- Ensure compliance with federal, state, and organizational regulations
- Participate in quality improvement initiatives and interdisciplinary committees
- Supervise and develop Social Services team members through coaching, training, and performance management
Required Qualifications
- Licensed Social Worker in the state of Colorado
- Master’s degree in Social Work
- Minimum of 3 years of experience providing clinical social services in long-term care, healthcare, hospice, or mental health settings
- Experience working within an interdisciplinary care team
- Strong knowledge of social services regulations in long-term care environments
- Excellent communication, documentation, and interpersonal skills
Preferred Qualifications
- Experience in senior living or skilled nursing environments
- Leadership or supervisory experience within a Social Services department
- Experience with discharge planning and care coordination
