Teams at The Center at Lincoln

Provides direct nursing care to residents in a skilled nursing and post-acute environment. Responsibilities include administering medications and treatments, coordinating with physicians and care teams, managing admissions and discharges, monitoring health changes, documenting care, educating residents and families, and supporting room readiness and orientation.

Assists with meal preparation, tray and food-cart setup, meal service, snack delivery, dishwashing, cleaning, food storage, and waste disposal. Ensures portion control, timely service, sanitation, infection-control, and workplace safety procedures are followed. Delivers meals and carts to designated areas, checks diet trays, maintains supplies, and reports hazards or incidents to supervisors.

Provides hands-on care to residents, supports comfort and dignity, communicates needs with the interdisciplinary team, contributes to care plans, and assists with daily activities. Requires compassion, patience, teamwork, strong English communication, physical stamina, and flexibility for varied shifts, weekends, holidays, and emergency call-backs.
