Perform insurance authorizations and reauthorizations, check eligibility/benefits, maintain electronic authorization records, enter authorization data into EMR, run departmental reports, verify payer setup, and communicate with internal and external stakeholders to ensure accurate authorization use.

Coordinate care and operations for a regional home health patient population: manage scheduling, staffing, recruiting, referrals/intake, insurance eligibility, payroll verification, and on-call duties. Work with clinical leadership to meet financial and operational goals, ensure regulatory compliance, maintain patient and employee confidentiality, and support special projects.

Provide and oversee nursing care for assigned home health patients: develop and implement individualized care plans, perform initial and ongoing evaluations, document required datasets (OASIS, PAR), coordinate care with providers and family, supervise ancillary staff, administer medications, attend case conferences, and ensure compliance with agency, state, and payer requirements. Maintain licensure, CPR, timely documentation, and participate in quality assurance activities.
