Provides trauma-informed case management to homeless or at-risk Veterans and their households. Responsibilities include assessing needs, coordinating housing and community services, conducting home visits, arranging transportation, documenting services in Caselogic and HMIS, evaluating financial assistance, supporting discharge planning, and maintaining confidentiality. The role requires collaboration with Veterans, families, community agencies, and SSVF team members, along with occasional travel throughout Colorado.

Provide trauma-informed case management and cost-shared rental assistance to very low- and extremely low-income Veterans to promote housing stability. Conduct assessments, develop housing stabilization plans, coordinate community resources, maintain detailed client documentation in Caselogic and HMIS, perform outreach and landlord engagement, and occasionally travel across Colorado. Ensure compliance with SSVF and VA program requirements.

Provide administrative and accounts payable support to the Finance department including A/P data entry, vendor maintenance, 1099 processing, check and ACH processing, print/mail large check runs, enter client check requests, assist with audits and special projects, and cross-train on general data entry tasks while maintaining internal communications and confidentiality.

Provides case management for individuals with developmental disabilities by reviewing applications and records, determining eligibility for services, preparing determination reports, coordinating interdisciplinary teams, maintaining compliant documentation, and explaining decisions and appeal processes. The role involves communicating with clients, families, providers, educators, and medical professionals; supporting care plans and service agreements; staying current on regulations; conducting face-to-face assessments; and managing multiple priorities while maintaining confidentiality and ethical standards.

Provide Medicaid and state-funded long-term services case management: assess eligibility, complete in-person assessments and service plans, coordinate providers and resources, manage caseloads and prior authorizations, monitor incidents, and communicate with members and care teams to support community-based living.
