Provide afterhours hospice nursing care including telephone triage, crisis response, admission/death/recertification and routine home visits, assess and document patient and family needs, and ensure timely eligibility evaluations and biweekly reassessments.

Provide full-scope primary care including diagnosis, treatment, preventive care, procedures, medication management, ordering tests, care coordination with specialists, documentation in Epic EMR, and supervise PAs/NPs as needed. Occasional coverage at a second clinic required.

Lead financial modeling and unit cost management for provider network contracting. Analyze utilization, evaluate network and contract financial impacts, set targets, produce reporting, advise cross-functional partners, and support negotiations with data-driven recommendations.

Supervise and coordinate clinical home health services, manage interdisciplinary staff, ensure continuity of care and regulatory compliance, handle referrals, insurance/benefit determinations, review plans of care, communicate with clinicians and physicians, and support individualized patient care in home settings.

Lead development and execution of provider network contracting and pricing strategies to manage unit cost and trend. Oversee financial modeling, network configuration, incentive-based payment models, and a team supporting client relationships across multiple markets to achieve synergy and access goals.

Provide full-scope outpatient primary care as a flexible NP or PA across Optum Denver clinics: manage 18–20 patients daily, document care, prescribe medications, order tests, coordinate referrals, and cover PTO/LOA/vacancies.

Provide in-home nursing assessments, develop and implement individualized care plans, coordinate care with an interdisciplinary team, facilitate transitions of care, refer to community resources, counsel patients and families, document thoroughly in the electronic medical record, and participate in quality initiatives. Frequent travel to patient homes required.

Provide 24/7 telephonic/telehealth advanced practice care to patients across settings, perform comprehensive assessments, document per CMS requirements, coordinate transitions of care, manage acute/chronic/behavioral conditions, and participate in quality and program initiatives while meeting shift and privacy requirements.

Welcome and screen patients and visitors, answer and route phone calls, schedule appointments, document patient communications in the electronic medical record, handle mail and shipments, maintain inventory and a clean reception area, run reports and provide administrative HR support, and perform other clinic administrative tasks under the Clinic Manager or COO.
