Provide on-site Physical Medicine & Rehabilitation (PM&R)/physiatry services at a skilled nursing facility. Collaborate with interdisciplinary teams (PT/OT/SLP, PCPs), track therapy progress, manage rehab-related conditions, attend meetings, and support facility education and culture-building.

Provide PM&R/Physiatry consultative care at a designated skilled nursing facility, collaborating with interdisciplinary therapy and PCP teams to optimize rehabilitation, manage common SNF conditions, follow therapy progress, address barriers to therapy, attend multidisciplinary meetings, and support facility education and culture-building activities.
