Colorado PACE

Colorado PACE is a nonprofit, community-based senior healthcare organization serving nursing-home-eligible older adults in Colorado. Through the Program of All-Inclusive Care for the Elderly, it coordinates personalized medical care, rehabilitation and therapy, in-home support, transportation, day-center activities, and social services. An interdisciplinary team manages participants’ care around their individual needs, helping them remain as healthy, independent, and safely at home and in their communities as possible.
