The Pharmacy Liaison assists patients with medication pick up, schedules appointments, processes medication orders, and maintains patient records in the pharmacy operating system.

Provide case management and outreach to people experiencing homelessness, develop and implement care and crisis plans, coordinate referrals to housing and health services, track client outcomes and maintain EHR documentation, transport and accompany clients to appointments, and collaborate with community partners using trauma-informed, harm-reduction, and housing-first approaches.
