Provides comprehensive case management for home-based patients, including care-plan development, clinical oversight of CNAs, family education, provider coordination, documentation, escalation management, quality improvement, and on-call coverage. The role requires local patient travel, flexible scheduling, and collaboration with multidisciplinary healthcare teams to improve outcomes, caregiver satisfaction, and readmission rates.

Owner of territory growth through consultative relationships with Case Management Agencies and individual case managers. Drive high-volume referrals via field-based outreach, education, pipeline management, and close collaboration with ops to convert referrals into admissions.

Coordinate Colorado case managers and agencies to manage service authorizations, care plan updates, reauthorizations, waiver transitions, and appeals. Ensure Medicaid and HCPF compliance, prevent care lapses, resolve complex cases, build agency relationships, train staff, and improve authorization processing and approval metrics across LTHH waiver programs.
