Vivent Health Benefits Overview

Provide client-centered enrollment and benefits assistance for people with HIV/AIDS, coordinating insurance outreach, ACA/Marketplace enrollment, staff training, documentation in EPIC/Provide Enterprise, referrals to case management, and monitoring policy changes to ensure access to care.

The Case Manager coordinates comprehensive care for clients, assessing their medical and psychosocial needs and providing advocacy and support within the healthcare system to enhance their well-being.

Provide evidence-based individual, couple, and family psychotherapy for patients in an integrated HIV care clinic. Conduct intake, assessment, treatment planning, therapy, documentation, referrals, care coordination, and follow-up. Participate in supervision, staff meetings, trainings, administrative and billing tasks, on-call coverage, and community/provider collaboration. Support culturally sensitive, trauma-informed care and may obtain additional certifications as needed.
