The Certified Nursing Assistant (CNA) provides hands-on personalized care to residents, collaborates with the care team, and ensures comfort and dignity in a fast-paced environment.

Provide meal service and food preparation support, including serving meals, portion control, tray setup and delivery, dishwashing, cleaning, and assisting cooks. Maintain safety, sanitation, infection-control practices, proper food storage, waste disposal, and report hazards or incidents. Wear protective equipment when handling infectious waste.

Prepare and serve nutritious meals, set up and clean dining areas, assist with food prep, storage, and labeling, and follow proper food handling and safety procedures.

Coordinate resident care and discharge planning, complete psychosocial assessments, facilitate referrals and community resources, educate residents/families on benefits, document in PointClickCare, participate in interdisciplinary care planning, advocate for resident rights, and support regulatory compliance and quality improvement.

Perform patient placement referrals, assessments, develop and maintain care plans, maintain patient records, assist with audits, and perform other assigned case management duties.

Partner with the Director of Nursing to lead clinical operations, mentor nursing staff, ensure regulatory compliance and quality improvement, manage staffing and performance, support survey readiness and infection prevention, collaborate with interdisciplinary teams, and assume DON responsibilities when needed.

Prepare and maintain nursing schedules, ensure adequate staffing, coordinate hires and replacements, verify payroll hours, manage central supply procurement, inventory, receiving, and returns, and enforce procurement and storage procedures while working with management and nursing leadership.

Provide direct nursing care to long-term care residents including medication administration, wound care, vital signs, assessment, documentation in EHRs, care-plan collaboration, and participation in quality initiatives under physician/medical director oversight.

Provide food service and support for residents by preparing, serving, and cleaning according to dietary policies. Assist cooks, set up trays, maintain portion control and sanitation, deliver meals, and liaise with medical/nursing staff. Perform routine cleaning, dishwashing, and follow safety and infection-control procedures. May work varied hours and respond to emergency call-backs.

Assist the Director of Nursing in planning, organizing, and directing daily nursing operations in a high-volume skilled nursing facility. Supervise nursing staff, conduct rounds, monitor staffing and absenteeism, oversee medication administration and documentation, ensure regulatory compliance, update policies/procedures, report issues, and provide nursing care as needed.

Provide direct occupational therapy services under the supervision of a licensed OT to sub-acute skilled nursing patients with orthopedic, neurological, cardiopulmonary, and complex medical conditions. Work collaboratively on an interdisciplinary therapy team, follow physician referrals, and comply with applicable laws, regulations, and facility standards.

Provide direct physical therapy services to sub-acute inpatients in a skilled nursing and rehabilitation center, treating orthopedic, neurological, cardiopulmonary, and complex medical conditions. Work from physician orders, follow applicable laws and Center standards, and collaborate with an interdisciplinary therapy team. Weekend availability required.

Occupational Therapists are needed to provide evaluations, establish care plans, and ensure quality therapy services in compliance with regulations.

The Assistant Director of Nursing manages nursing operations, ensures patient care standards, supervises staff, and ensures regulatory compliance at a skilled nursing facility.

As a Physical Therapist Assistant, you will deliver treatment interventions, monitor patient progress, educate families, and maintain documentation under a supervising Physical Therapist.

The COTA assists in planning and directing Occupational Therapy Services under supervision, collaborating with staff and patients to optimize therapy outcomes.
