Provides peer support and transition assistance to adults with behavioral health or intellectual/developmental disabilities exiting incarceration. Coordinates care linkages involving housing, employment, education, transportation, benefits, treatment, and community services. Builds relationships with justice-system partners, meets recipients in correctional facilities and community settings, supports person-centered planning, and facilitates warm handoffs to ongoing care management and resources.

Provides peer support and transition assistance to adults with behavioral health or IDD conditions leaving incarceration. Educates recipients and guardians, coordinates care and community linkages, supports housing and service access, and addresses reentry barriers such as transportation, benefits, food, and clothing. Builds relationships with correctional facilities, probation and parole, and community providers while facilitating person-centered transition planning and warm handoffs.
