Provides administrative support to the Director of Operations through data entry, document preparation, scanning, faxing, spreadsheet and log maintenance, phone support, reporting, and project assistance. Maintains confidentiality, communicates professionally with internal and external contacts, submits documentation on time, and supports People Services and Operations staff. Requires strong organization, multitasking, problem-solving, computer, and written and verbal communication skills. A high school diploma or equivalent is required.

Provides home health case management as a registered nurse, including patient assessments, individualized care plans, medication administration, documentation, care coordination, patient and family education, supervision of ancillary staff, physician communication, and on-call triage. Ensures compliance with state, federal, payer, agency, infection-control, and quality requirements while delivering skilled nursing care in patients’ homes.

Provides home health nursing case management for assigned patients, including assessments, individualized care plans, medication administration, documentation, care coordination, patient and caregiver education, and supervision of ancillary staff. The RN communicates patient changes to physicians, participates in quality assurance activities, handles on-call triage, and ensures compliance with clinical, regulatory, and agency requirements.

Oversee an assigned home-care caseload and census, ensuring staffing coverage, scheduling, recruiting, and continuity of care. Communicate with families and field staff, coordinate with nursing and case management teams, support referrals and intake, verify payroll, address insurance eligibility and authorizations, and help maintain branch financial performance and regulatory compliance. Participate in an after-hours on-call rotation and manage staffing gaps proactively.

Coordinates home health operations and patient care, including scheduling, staffing, recruiting, intake, referrals, insurance eligibility, authorizations, payroll verification, on-call coverage, compliance, and financial performance. The role collaborates with nursing leadership, case managers, clinical staff, patients, and employees while maintaining confidentiality and meeting operational deadlines.

Perform insurance authorizations and reauthorizations, check eligibility/benefits, maintain electronic authorization records, enter authorization data into EMR, run departmental reports, verify payer setup, and communicate with internal and external stakeholders to ensure accurate authorization use.

Coordinate care and operations for a patient population in a regional long-term home health division. Manage scheduling, staffing, recruiting, intake/referrals, insurance authorizations, payroll verification, on-call coverage, and collaboration with nursing and case management. Support financial goals, ensure regulatory compliance, maintain patient and employee confidentiality, and complete special projects as assigned.

Provide and oversee nursing care for assigned home health patients: develop and implement individualized care plans, perform initial and ongoing evaluations, document required datasets (OASIS, PAR), coordinate care with providers and family, supervise ancillary staff, administer medications, attend case conferences, and ensure compliance with agency, state, and payer requirements. Maintain licensure, CPR, timely documentation, and participate in quality assurance activities.
