Manager, Care Management Education and Development
The Manager of Care Management Education & Development will oversee the design and delivery of educational programs and training to ensure the successful on boarding and learning of clinical teams. Clinical teams variably consist of nurse practitioners, registered nurses, registered dieticians, medical assistants, care coordinators, social workers, pharmacists, pharmacy technicians, and others. The education programs and training will encompass evidenced based clinical practice guidelines and adult learning theory. The Manager will project manage and direct the work of others in developing robust programs that meet the training and continuous learning needs of new hires and current/future employees. This position will work collaboratively with multiple teams to include Market Operations, Product Development, IT, Quality, and Clinical Teams. This position will report to the Director, Clinical Education.
Essential Functions

  • Design training programs for clinicians engaged in the comprehensive Care Management of CKD and ESRD patients.
  • Conduct and oversee workflow analysis and processes to ensure programs are patient centric and staff performance is in line with policy and procedures
  • Identify opportunities for improvement in the education and learning process and implements strategies to improve clinical excellence.
  • Serve as the subject matter expert on technical applications such as electronic health records and platforms utilized for provision of care, care coordination and reporting.
  • Facilitate completion of patient goals through a multidisciplinary approach of collaboration with the Strive Care Team, external resources, and family members
  • Serves as a Subject Matter Expert in regard to principles of clinical nursing, case management and NCQA and CMS Standards
  • Collaborate with all levels of the clinical and operational teams to improve processes, communication, and team skills needed to provide the best care to our patients
  • Develop and execute the program requirements for on boarding new staff members as well as meeting the educational needs of the clinical staff in a live and virtual venue.
  • Ensures education programs and training modalities are evidence-based and supportive of adult learning theory.
  • Practices excellent employee relations with attention to employee needs and develops and mentors a high performing management team for all areas of responsibility. Mentor clinical educators and key field super trainers in professional development.
  • Creates and analyzes reports to identify opportunities for improvement in the education and learning process and implement strategies to improve clinical excellence.
  • Design and provide the ongoing education and training with in-market preceptors.
  • Travel may be required for education delivery, learning assessment, and hands-on competency training.
  • Meet in person with internal and/or external stakeholders to facilitate team and business priorities/opportunities.


Qualifications
Minimum Qualifications

  • Bachelors in nursing, with active RN license.
  • 5 years' experience as a registered nurse with experience precepting students and/or new clinical hires
  • 3 years of recent didactic teaching experience, including curriculum development
  • 3 years' experience of management/leadership experience within a healthcare related field
  • 2+ Nephrology or Chronic Disease management experience required
  • 1+ years of project management in the delivery and execution of educational programs
  • Certification by a nationally recognized case/care management organization
  • Business travel


Preferred Qualifications

  • MSN or Doctor of Nursing Practice and/or Doctor of Education Degree
  • Complex case management experience in a managed care environment preferred
  • Working knowledge of evidence-based practice guidelines in the development of care plans
  • Created curriculum around CMSA developed Standards of Practice for Case Management
  • Previous experience with CKD and ESRD patients preferred
  • Strong organizational skills with ability to handle multiple competing priorities
  • Excellent communication skills both verbal and written, strong public speaking skills
  • Strong technical skills including proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and platform for charts/diagrams (LucidChart)


Annual Salary Range: $97,000.00 - $119,000.00
